JHI Get Fit! Recurring Registration for Current Members

Welcome Back! Thank you for continuing with JHI and working on your personal journey towards being Fit and Healthy as you age!
This form is for participants who are continuing monthly enrollment with JHI and do not have a break in enrollment for more than two months. JHI members returning after two months or longer need to re- enroll using the NEW REGISTRATION link, re-sign the waiver and provide contact information again. Please note registration for current members will be open up two weeks prior to upcoming month of session cycle. 
Please complete this form and submit with the required payment. JHI will send you confirmation of enrollment. If you are owed any credits, JHI give back rewards, other discounts, or prorated amounts to be applied to your payments, JHI will inform you prior to your re-enrollment and payment. There is no membership contract or fee, and no refund of payment after enrollment.

PROGRAM KEY NOTES
PLEASE NOTE THE FOLLOWING:
  • If you are enrolling in the sessions after the start of JHI monthly cycle, you will pay the monthly fee and it will apply from the day you start attending the session for 4 weeks approx. 23 sessions. The following month when JHI next 4 weeks cycle starts, you will pay a pro-rated amount for the remaining unpaid days in the cycle. JHI will notify you of the payment due and process of payment prior to start of the next cycle and prior to your attendance.
  • There will be no refunds after enrollment and first day of attendance. All sessions will run as per monthly schedule. JHI follows the U.S calendar for public holidays and will be closed accordingly.  Any sessions scheduled and paid for, that is cancelled by JHI will be re-run as a makeup class, or payment for the cancelled class will be refunded to participant in the time agreed upon by both JHI and participant. JHI reserves the right to change schedule, format, policies and pricing of sessions. All members will be notified a month in advance prior to such changes and prior to any monthly payment to allow ample time for members to make decisions regarding further participation in JHI programs.
  • If you are enrolled in weekday sessions, you can make up the morning sessions in the evening and vice-versa. Doubling up sessions on same day to make up is not recommended.  There is no makeup for Saturday sessions.

To Register Please Complete the Following and Submit: 

STEP 1: Please enter the required information
STEP 2: Please select or enter the date you will start attending the session
STEP 3: Select your primary sessions timing i.e., your preferred timing to attend sessions on most days. Note: You can make up a session that you missed by attending morning or evening based on your primary selection. Note: Saturday session is at 8: 45 a.m. Contact JHI to evaluate your readiness for this intermediate/advance session

STEP 4: Please select your payment months
Select monthly or multiple months payment from the provided options. The pricing shown is the flat fee per month. Any discounts or JHI give back reward amounts to be considered will be applied at the time of remitting payment via Zelle. You can reach JHI to clarify the fee to remit if JHI has not already informed you prior to your registration.  If you have paid multiple months but change your mind or JHI changes the pricing etc. for following months, you can ask for refund. Refund will only be issued for months not attended, if you have paid multiple months, or JHI cycle has not started, and you choose not to attend further sessions.

Please note if payment is made by someone else on your behalf, please put your full name in the notes section of your Zelle payment. Also, all members please put the start date and total amount paid (if discounted) in the Zelle note. PLEASE NOTE VERY IMPORTANT: YOUR REGISTRATION IS INCOMPLETE IF PAYMENT IS NOT MADE AT THE SAME TIME OF REGISTRATION SUBMISSION

STEP 6: FINAL CHECK BEFORE SUBMISSION OF REGISTRATION
Please make sure that you have filled the required fields and made payment via ZELLE, prior to clicking the submit button.
Upon successful registration JHI will receive notification and JHI will send you onboarding information, the associated zoom link for you to log into your registered sessions along with a weekly schedule for reference, and a note on equipment that may be required for the workouts, and you will be automatically added to JHI communication group and OWEN network group.
